Prejuvenation: Preserving Youth Before Wrinkles Begin

One of the most noticeable trends I am seeing in aesthetic medicine today is the use of injectable treatments to delay — and in some cases prevent — the development of facial wrinkles. This proactive approach, often called “prejuvenation,” has become increasingly popular among younger patients, typically those in their 20s and 30s, who are interested in maintaining a refreshed, youthful appearance over time rather than correcting more advanced aging later.

Botox has been FDA-approved for the treatment of facial wrinkles since 2002 and is commonly used to soften frown lines (the “11s” between the eyebrows), forehead lines, and crow’s feet, along with other areas of facial expression. Dermal fillers have also been used for many years, and since the early 2000s, hyaluronic acid fillers have become the leading products in this category due to their safety, versatility, and natural-feeling results.

When used thoughtfully, Botox can help prevent expression lines from becoming permanently etched into the skin by reducing repetitive muscle movement over time. Fillers, in contrast, can help maintain or subtly enhance facial structure and support — including the lips, cheeks, chin, and jawline — preserving balance and proportion as the face matures.

As patients age, wrinkles often become deeper and more established. At that stage, they can be more difficult to fully correct, sometimes requiring higher doses of Botox or filler while still not achieving complete improvement. Younger patients, however, usually do not yet have deep wrinkles at rest. Lines appear primarily with facial expression, and this is where preventive treatment can be particularly effective — minimizing repetitive folding of the skin before those lines become permanent.

Equally important is the quality of the result. When injectable treatments are performed conservatively and with expertise, patients do not appear overfilled, unnatural, or “frozen.” Facial movement and expression remain intact — you can still smile, laugh, and show emotion — while the overall appearance looks smoother, more rested, and naturally youthful.

The goal of prejuvenation is not to change your face. It is to preserve your natural features, allowing you to age gradually and gracefully with subtle support along the way.
